Driver’s License

Need to get a Florida driver license or renew your license?

  • A mandatory vision test is required for all drivers over the age of 80, regardless of citizenship or status, or renewal method.
  • Customers who are 79 years of age and under will be issued an eight year license.
  • Customers 80 and older will be issued a six year license.

Effective January 1, 2004, all drivers who are 80 years of age or older and who are in the process of renewing their driver license are required to pass a vision test.

The test may be taken at Tax Collector’s Office in Polk County at no additional charge.

Your other option is to have a doctor licensed to practice in Florida administer the vision test. He or she must be a medical doctor, osteopathic physician, or an optometrist. If the vision test is administered by your doctor a Mature Driver Vision Test Form (HSMV 72119) must be completed and submitted to the department. The doctor completing the form must be licensed in Florida or licensed at a federally established veteran’s hospital. Out-of-state doctors are not authorized to provide this information.

Upon passing the vision test, you can complete the renewal process. However, if the test reveals a problem, you will be requested to have an eye specialist licensed in the state of Florida complete a Report of Eye Examination (HSMV 72010) and submit it to the department prior to renewing your license.

Questions & Answers:

  • I am blind in one eye. Will I lose my license?
  • o Just like any other driver, if you are blind in one eye, the other eye has an acuity reading of 20/40, and your combined vision is 20/40, you will be eligible to receive a driver license.
  • What eyesight (acuity) readings must I have to receive a driver license?
  • o Applicants or licensed drivers who have 20/50 vision or worse in either eye, with or without corrective lenses, are referred to an eye specialist for possible improvement. 20/70 vision or better in either eye or with both eyes may pass with or without corrective lenses, if vision cannot be improved. However, if one eye is blind or 20/200 or worse, the other eye must be 20/40 or better. The minimum acceptable field of vision is 130 degrees.
