INSTRUCTIONS
exempt county local business tax receipt can be renewed by way of email if there
are no changes to the account.
Any changes in ownership, business name, location, mailing address, activities,
etc., need to be documented in person at anyone of the Tax Collector's
Service Centers or can be accomplished through the mail (for more information
review the renewal materials mailed to you or telephone 863-534-4700).
By sending an email to the Tax Collector's Office to renew a county local business
tax receipt, the holder of the tax receipt makes the following Application Affidavit:
I have read, understand and acknowledge the following: This is an application for
an annual county local business tax receipt and that by sending an email to renew
an exempt business tax receipt I swear the information provided on this application
is current, true and correct; The privilege to conduct business in Polk County,
Florida is limited to the activities, professions and/or occupations identified
on the county local business tax receipt; A local business tax receipt does not
waive local, state or federal laws and/or requirements, including any licensing,
registration, and/or certification requirements; The holder of a local business
tax receipt must adhere to all local, state and federal laws and requirements, including,
but not limited to, local codes and zoning restrictions; The applicant is required
to provide a Federal Employee Identification Number or Social Security Number to
the Tax Collector pursuant to F.S. 205.0535(5); The applicant will only do business
under their legal name or has provided a fictitious name registration number to
the Tax Collector or will not engage in business until a fictitious name registration
number is obtained from the Florida Department of State, Division of Corporations;
The holder of a county local business tax receipt must report changes in business
name, ownership and/or location, as well as any changes affecting their status as
a county local business tax receipt holder, to the Tax Collector; and, Providing
false information on this application is a criminal offense subject to prosecution.

exempt county local business tax receipt, your County Local Business Tax Receipt
Number and a contact telephone number.
