Name Changes

 If you are coming in to change your name, you must first update with the Social Security Administration Office.

Note: Please allow 24 to 48 hours after changing your name with Social Security Administration before scheduling your Driver License or Identification Card appointment.

An original or certified copy of all marriage certificates or court orders linking name change(s) from birth to current name. You must submit the original or a certified copies of the document.

This is only required when a prior name (or maiden name) is shown on the identification document provided.

A U.S. passport in your current legal name may also be presented.

What to Bring

Photocopies are not accepted. Certified copies of marriage certificates or divorce decrees may be obtained at the clerk of court in the county in which you were married.

Church-issued marriage certificates are not accepted.
